Homeless Management Information Systems (HMIS) Class Registration

  1. To Register for a Class

    Please submit one form for each registration request. Forms should be completed by the Agency Administrator for your agency. Once registered, you will receive a confirmation email within five business days with the location and parking information. If a class is full, you will be notified of the next available date.

    Registration forms must be completed by the Agency Administrator - select one category from the following:

  3. New HMIS End User Training

    *After you submit this training request, if you are a Coordinated Entry or PATH end user, you must also submit a separate Special Training Request for additional training on required data elements. This training must take place after your New HMIS End User Training, but you may submit your Special Training Request now.

  4. Agency Administrator

    *Must complete New HMIS End User Training as a prerequisite.

  5. Reports Training

    *Must complete New HMIS End User Training as a prerequisite.

  6. Special Training: Coordinated Entry, PATH, Outreach, and Refreshers

    *Must complete New HMIS End User Training as a prerequisite.

  7. List the name of the provider, as listed in ServicePoint, for which this user will be set up.

  8. List the name of the project, as listed in ServicePoint, for which this user will be set up.

  9. List the name(s) of the project(s), as listed in ServicePoint, to which this user should have access.
